COGNITIVE REHABILITATION PROGRAM

Our cognitive rehabilitation program is person-centered and function-oriented in which individuals experiencing cognitive deficits are assessed, evaluated, and treated to improve their independence in their day-to-day lives and meaningful occupations. Treatment is tailored to each individual and designed to meet their unique cognitive challenges, most commonly due to an acquired brain injury.

Our cognitive rehabilitation program utilizes the strengths of a multidisciplinary approach, involving the coordination and interventions from occupational therapists, physiotherapists, speech and language pathologists, rehabilitation support workers, and many more. In addition, our treatment approach collaborates with neurology and neuropsychology to ensure the most impactful care.

WHO CAN ENROLL

Anyone experiencing cognitive difficulties. Cognitive rehabilitation is recommended for individuals who have experienced an acquired brain injury or a concussion and have problems with their cognitive function. Common cognitive issues we address through our cognitive rehabilitation program are; impaired attention, decreased concentration, communication disorders, poor executive function, impaired memory, and more.

EXCLUSION CRITERIA

This program of care has some exclusion criteria, which are significant red
flags that may prevent the individual from being eligible to partake in this program. Please consult a healthcare practitioner or contact us to confirm eligibility.

WHAT CAN I EXPECT ON MY FIRST VISIT

Your first visit would consist of an initial interview with one of our Occupational Therapists, who will use a functional and client-centered approach to assess and evaluate your cognitive function through peer-reviewed and standardized assessments, establish rehabilitation goals that reflect your values and interests, and create an individual treatment plan for you.

Our Occupational Therapists will use specialized assessments throughout your rehabilitation to evaluate the effectiveness of treatment to support our clients throughout their rehab process. Some common estimates that our occupational therapists are trained to use include;

  • BrainFX Screen and BrainFX 360 Assessment
  • Test of Everyday Attention
  • Cognitive Linguistic Quick Test
  • The Multiple Errands Test
  • Rivermead Behavioural Memory Test-Third Edition (RBMT-3)
  • Behavioral Assessment of the Dysexecutive Syndrome (BADS)

The healthcare provider will also assess for possible Yellow flags, which can indicate psychological risk factors that may impact their recovery due to behavior, expectations, and recovery from injury. Finally, a discussion on achievable goals from the treatment program will be discussed as well as the responsibilities of the person served and health care provider throughout the program.

WHEN WILL I BE DISCHARGED

The duration of the Cognitive Functional Program is determined after the initial assessment and depends on each individual’s needs.

To be discharged from the Chronic Pain Program, one or more of the following criteria must be met:

  • The person served shows a normal range of motion, strength, and function.
  • There is no evidence of pain or other subjective complaints.
  • The person served has achieved their goals with the rehabilitation team.
  • The services of skilled therapists are no longer necessary or appropriate.
  • The person’s progress has slowed or temporarily stopped, and they are no longer benefiting from the program’s services.
  • The rehabilitation program is “put on hold” due to prosthetic fittings, resolution of medical problems, pending surgical intervention, or the arrival of specialized equipment.
  • It is the therapist’s conclusion and consensus that other programs or levels of care and treatment are more appropriate for the person served.
