Health Bound Health Network is a CARF accredited, multidisciplinary clinic and resource center that serves over 45 communities across Ontario. Our Occupational Therapists (OTs) address mental, physical, cognitive, developmental, emotional, and environmental barriers to enable engagement in their meaningful activities. Our team of OTs are skilled in providing hospital discharge planning services which often includes a pre-discharge home safety assessment and recommendation of home modifications or assistive devices, such as, grab bars, wheelchair ramps, stair-lifts to maximize the clients’ safety and independence in their home. Our OTs are also experienced in providing vocational support to their clients with return to work, by assessing the client’s work environment/demands and creating customized plans to help them return to their previous or modified work duties. Health Bound OTs also help their clients rediscover their leisure occupations and find ways to help them participate in the activities they love or find new activities that they can enjoy, such as sports, gardening, etc. Last but not least, our OTs specialize in the completion of attendant care assessments, situational and catastrophic assessments and more for our clients who have been involved in a traumatic accident and require support from an occupational therapist to return to their previous level of function or to maximize their function despite new limitations caused by a traumatic injury.
Health Bound has highly trained CAT certified Occupational therapists working with Psychiatrists, Neuropsychologists, Orthopaedic Surgeons, Neurologists, Physiatrists, and other healthcare professionals.
