What the Program Entails

Health Bound Health Network has a specialized, interdisciplinary, coordinated, and outcomes focused program designed for individuals with an acquired brain injury, including strokes. Our multi-disciplinary team of health care providers address the unique medical, physical, cognitive, psychosocial, behavioural, vocational, educational, and recreational needs of people with different types of acquired brain injury. The program encompasses care that enhances the lives of the people served within their families/support systems, communities, and life roles.

Who Can Enrol?

-Diagnosis of non-progressive mild-moderate-severe brain injury

-Requires rehabilitation and support based on individualized functional goals

-Medically stable, NOT hospitalized

-Ability to transfer and mobilize independently (with or without mobility aids)

What Can I Expect On My First Visit?

An initial assessment for treatment planning is conducted by a Health Care Provider at the start of the Program of Care. The assessment includes the taking of a complete history, subjective evaluation of the primary complaints related to the injury, a thorough physical examination including range of motion evaluation, orthopaedic testing, muscle testing, and neurological testing. The Health Care Provider will also administer outcome measures to obtain baseline measures of pain and function. These may include, but are not limited to the Numeric Pain Rating Scale (NPRS), the Oswestry Disability Index, the Neck Disability Index (NDI), the Lower Extremity Functional Scale (LEFS), or the Upper Extremity Functional Scale (UEFS).

The Health Care Provider will also assess for possible yellow flags, which can indicate psychological risk factors that may have an impact on their recovery as a result of behaviour, expectations and recovery from injury. Discussions on achievable goals from the treatment program, the responsibilities of the person served and the Health Care Provider will be held through the duration of the program.

Our Multi-disciplinary Team of Health Care Providers

Our multi-disciplinary team of health care providers consists of specialists from a number of disciplines to obtain a holistic view of each patient’s condition. Our team consists of, but is not limited to neuropsychologist, neurologist, occupational therapist, physiatrist, psychologist, speech language pathologist, social worker, rehabilitation support worker, personal support worker, physiotherapist, case managers. Our multi-disciplinary approach ensures continuity of care for patients, and most importantly, makes the treatment if patients more effective and shorter in duration.

Duration Of The Program

The duration of the Program of Care is determined after the initial assessment and is dependent on the needs of each individual.

Exclusion Criteria

The above eligibility criteria exclude people who have significant red flags such as Neurological deficit (major motor weakness, disturbance of bowel or bladder control), Infection (fever, urinary tract infection (UTI), intravenous (IV) drug use, immune suppression), Fracture, Tumour.
