Hospital Discharge Planning
At Health Bound, we have created a reliable discharge – coordination and follow- up process. Often times, patient discharge is managed poorly resulting in patients leaving a hospital without key information about their care, and without having a follow-up appointment arranged. This poor coordination at discharge can lead to readmission to a hospital and adverse health outcomes. Our Case Managers visit individuals who are in hospital and liaise with hospital staff to facilitate the discharge process, and improve the transition from hospital to home or other health care facilities.
Home Healthcare Planning
Case Managers ensure that the home environment is assessed prior to the discharge of a person, that appropriate modifications are completed, and assistive devices are installed. Case Managers also arrange assessments to determine a person's need for devices to help them during their rehabilitation. Along side the interdisciplinary health care team, Case Managers help to facilitate treatment planning and personal assistance planning.
Case Managers liaise with Occupational Therapists, Physiotherapists, Chiropractors, Speech Language Therapists, Psychologists, Social Workers, Rehabilitation Therapists and other health professionals that may be involved to coordinate care delivery for the person served.
Personal Assistance Planning
Case Managers help to connect the person served with personal support workers to help assist with personal care and facilitate activities of daily living. Case Managers coordinate assessments of housekeeping, care-giving, and attendant care needs by a regulated health professional.
Life Care Planning
Case Managers summarize the medical, educational, vocational, psychosocial, and daily living needs of the person who can function indefinitely only with professional assistance. The Case Manager also projects the long term costs of care, and establishes rehabilitative goals while coordinating future care providers in order to best assure a continued care and facilitate recovery.
